Vendor:
Thank
you for your interest in being placed on the Vendor
Bid List for the City of Visalia. We would appreciate
your completing our attached
form so that we may update our Vendor Database.
It is very important that this form is filled out in
its entirety. The more we know about your company, the
more likely it will be that you receive an invitation
to bid in all the categories in which your company has
expertise. Also, there is certain information that the
City of Visalia requires prior to conducting any business
with your firm. Following is a list of that required
information:
- Federal
Taxpayer Identification Number
(The number issued to your firm by the Internal Revenue
Service under which you file your firm’s federal
income tax and federal payroll taxes.)

City of Visalia Business Tax Certificate Number (if
applicable to your company)

Contractor’s License and Classification (if
applicable to your company)
If
your Company has an e-mail address, please include that
information, as we will soon have the capability of
sending and receiving Requests for Bid by e-mail. Also,
if your Company has a web site, please give us that
information. In the "Categories Products &
Services" section list the major products and/or
services your company provides as this will help us
determine which projects are compatible with your company’s
expertise.
The City of Visalia frequently requires its suppliers
and contractors to maintain insurance covering them
against claims or judgments arising from their products
or activities for the City. Prior to commencing any
work with the City, your firm may be required to submit
original Certificates of Insurance evidencing the necessary
coverage. This insurance is to remain in effect for
the full term of any contract, or for as long as the
supplier is providing products or services to the City,
and is provided at vendor’s expense.
